Folding Instructions:

1. Print this page (landscape orientation)
2. Fold in half lengthwise (hamburger fold)
3. Fold in half again (same direction)
4. Fold in half one more time (hotdog fold)
5. Unfold completely, then cut along the center horizontal line between the middle two columns
6. Fold lengthwise again and push ends together to form the book
